During the course of your treatment at Hart and Bones, your personal information such as contact details and medical history will be collected and stored.
We need to have your contact details for appointment purposes and we need to securely store your clinical notes on the premises as a basic requirement of our clinical professional standards.
The staff at Hart and Bones will never pass any of your contact details to a third party and your clinical records will never be shared unless you have given written consent.
You have a right to see your treatment records and you have a right to have your personal data deleted on request. However, we are required by law to retain treatment notes.
You can also withdraw your consent to treatment at any time.
Clinical notes no longer in use are securely archived and destroyed after eight years. Child patient records are retained until eight years after they reach 18.
